Time to keep your indoor pollution under control!!

Winter has just knocked the door. And also, the gloomy and hazy weather.  The Air Quality Index is showing alarming levels (~900), categorized under “severe” levels. Schools are closed and kids are forced to stay indoors.

Install an air purifier.

Air purifiers are machines that suck the pollutants (including fine Particulate Matter, PM2.5 particles) from air in indoor settings. Earlier air purifiers were required for people with allergies and Asthma but it is fast becoming an essential need for a busy city household.

The number only suggests the purifier’s ability to remove pollutants like tobacco smoke, dust and pollen from specific amount of air. It has nothing to do with the effectiveness to filter very small particles (most harmful). Also, higher number does not indicate the better durability of purifier. CADR is often measured at highest setting of the purifier that is often impractical to keep at homes due to noisy operation. Look for high CADR along with specifications like HEPA filters and pre filters (often the expensive ones)

Why is the Measles vaccine given at 9 months? Is measles a common disease that early in life?

Measles is a highly contagious disease; spread by breathing, coughing or sneezing the viral infected respiratory droplets and direct contact with the diseased patient. Before the vaccines came into existence, more than 90% children used to be infected with this deadly viral disease. Measles can be severe with complications like Pneumonia and Encephalitis, which can be fatal in children less than 5 years of age. Since, infants are protected against Measles for 6-9 months only after birth, by the passive immunity attained through maternal antibodies. Measles vaccine has to be essentially given to all infants >9 months of age and young children for further immunity as a part of national immunization programs.

Does my baby need an additional Hep B dose at 6 months after previous doses of combination vaccines?

My baby had hexaxim at 6 week followed by pentavac pfs at 10weeks and  14 weeks. Should I still take her for HepB vaccine for the 6th month dosage?

Hepatitis B vaccination is recommended as 3 doses only. Timeline of the doses depends on the type of vaccine used. If it is a monovalent vaccine with only Hepatitis in it, Birth dose followed by a 2nd dose at 6 weeks and 3rd dose at 6 months of age (2nd and 3rd dose can be monovalent/pentavalent).

Alternatively as in your baby’s case, where the combination (pentavalent-pentavac pfs and hexavalent-hexaxim) vaccine including Hep B is chosen to be given, 3 primary doses of combination vaccine, is given at 6, 10 and 14 weeks after birth dose (monovalent).

Further vaccination is not required for Hepatitis B as the immune response of baby is primed already with the given doses. For details see footnotes of

Flu vaccine update 2017-18

Influenza vaccine composition needs to be changed each year after evaluating the genetic composition of circulating Influenza virus strains at that time. Influenza viruses continuously evolve and vaccine needs to be “smarter” each year to provide effective immunity against flu. WHO recommends the revised ingredients of the Influenza virus vaccine each year, based on the observations and evaluations of the various laboratories worldwide. Scientists at the National Institute of Virology (NIV) in Pune, run by the Indian Council of Medical Research’s (ICMR), isolated a new strain called the Michigan strain circulating in Maharashtra during the H1N1 flu monitoring. According to Dr Soumya Swaminathan, director-general ICMR, old Influenza vaccine may be ineffective against the new strain of swine flu.

2017-18 recommendations by WHO for the trivalent Influenza vaccines to be used in northern hemisphere including India are:

  • an A/Michigan/45/2015 (H1N1)pdm09-like virus;
  • an A/Hong Kong/4801/2014 (H3N2)-like virus; and
  • a B/Brisbane/60/2008-like virus.
So, how are the ingredients different this time?

The H1N1 (swine flu) pdm09 virus has been updated from California strain (or type)  virus to Michigan strain (or type) virus. Other virus strains should remain the same as in last year vaccine concoction.

The new stocks of Influenza vaccine are likely to hit the Indian market by mid august or early September. Please get the vaccination done if you or your family member belong to high risk group as soon as the new vaccine is available. Children from 6 months to 9 years of age are susceptible to Influenza and can be given vaccine each year. Pregnant women should be given the vaccine in 2nd or 3 rd trimester as they are highly susceptible to serious flu infection. Vaccine will not only protect the mother-to-be but also her newborn baby later.  Adults coming under high risk category are patients with Lung, Liver, Heart and Kidney disorders, Diabetes, hematological diseases, diseases that impair respiratory functions, and immunosuppression. Also, the personnel employed in nursing homes or health care facility and household contacts of children <5 years are recommended to take the yearly flu shots.

So, absolutely no need to opt for painless vaccine if your baby is healthy and can tolerate slight discomfort with your  love and care.

Pain and fever are associated with whole cell pertussis antigen in DTwP vaccines and it is not of major concern as they can be easily managed with paracetamol drops. Also,  immunity provided is better than the painless acellular Pertussis antigen containing DTaP vaccines. Primary vaccination should be preferably done with wP containing combination vaccines. In west, Pertussis breakout after prolonged use of DTaP vaccines should be lesson well learned for us to stop using painless vaccines and instead opt for cheaper and easily available DTwP vaccines.

Painless vaccines like earlier Pentaxim (2275 Rs.) and now Hexaxim (cost 4800 Rs.) are heavy on pocket due to the purification techniques used to produce them. Hexaxim is double the cost than Pentaxim as it is the only vaccine in market which gives protection against 6 diseases in one shot and also it is ready to use and preservative free. Choose Hexaxim if you care for no. of pricks to your baby.
In your baby’s case, if he tolerated Quinavaxem well, there is absolutely no need to change the vaccine.  Regular DTwP vaccines like Quinvaxem and Easy five costs less than thousand bucks and provide effective and better immunity. Polio vaccine cost maximum of 500 Rs.  So, now you are a better judge to decide, which vaccine is best for your baby.

Why do we need to give Polio vaccine to children every year till the age of 5?

Polio virus is a very infectious virus which spreads through fecal to oral route in countries where sanitation facilities are poor. Wild polio virus gains entry inside a susceptible child through fecal contaminated food. Virus then multiplies in the intestine of the infected child, who keep shedding it continuously through feces and in between can make others sick too. So, in case of Polio, herd immunity plays a major role . It is important to deprive the virus of susceptible hosts through vaccination. This can be done only, when the entire community is immune to the virus. It mainly causes paralysis in the growing children. Measles Rubella vaccination drive across India!! Should you vaccinate or not vaccinate your child?

The measles-rubella (MR) vaccination drive was launched on Feb 6 and it will go on till Feb 28 throughout India. It will target more than 35 million children between the ages of 9 months to 15 years. According to WHO, India has taken a major big step towards reducing childhood mortality and birth defects through measles- rubella vaccination campaign.

The initiative will work to ensure that no child dies from measles or will be born with congenital rubella syndrome (CRS) by 2020. The campaign will be followed by inclusion of MR vaccine in the Routine Immunization program. Vaccine is 100% safe and parents should utilize the opportunity to vaccinate their children and should not believe the rumors about the side effects of vaccine. Though, parents should inform the school authorities in advance in case of any present illness which may make the child vulnerable to side effects of any medicine or vaccine given. Healthy children are perfectly fit to take the vaccine without any side effects.

Measles is one of the most contagious diseases worldwide and is responsible for death and disability among children. Rubella is an under recognized public health problem as it usually causes mild rash illness in children and adults. But, Congenital rubella syndrome (CRS) is a major disease, which can occur, in a developing fetus of a pregnant woman who has contracted Rubella virus infection usually in the first trimester. Rubella infection during pregnancy can lead to serious consequences such as miscarriages and stillbirths. Rubella vaccine is a live virus vaccine and cannot be taken during pregnancy. Females of childbearing age should be vaccinated with rubella vaccine to avoid complications of rubella during pregnancy. It is advised not to get pregnant before one month has passed after MMR vaccination. Vaccinating the majority of the population is by far the most effective way to prevent congenital rubella syndrome.

In government hospitals, Measles was only routinely given to infants and school going children. Rubella vaccine was not included in Routine immunization program. Though, private clinics were administering the combination vaccine MMR (Measles Mumps and Rubella) already. Then, question arises that why is vaccine given to every child irrespective of his/her immunization history? The answer to this lies in the fact that most of the immunization records are incomplete or sometimes even lost by the time, child turns 6 years. In many records, brands of vaccine or whether measles alone or MMR vaccine is given, is also not mentioned. So, there is no way to find, that a child has completed the required doses of MR/MMR vaccine or not. India really needs a web based immunization registry to know the status of immunization of each child after every visit. Otherwise, resources, time and overall health will be jeopardized every time when the urgent call of action is needed.

India has shown its potential of tackling health problems by eradicating Polio through successful pulse polio program. It is utmost important to identify the infected and vulnerable areas of populations and make the vaccine available to each child there. Proper surveillance and monitoring of vaccination drive will be needed.
